The Family Medicine Center, in partnership with the Fort Wayne Medical Education Program, has been serving the Fort Wayne community since 1967. During this time, we have developed a proven Patient Centered Medical Home model, built on the foundation of passionate, evidence-based care. We are committed to providing each member of our patient family with the high-quality care they deserve.
Our team of nurses, residents, and faculty are standing by to provide a wide range of services, resources, and classes. We also have established relationships with all three hospitals in Fort Wayne and several in the surrounding areas. If needed, our Physicians can refer our patients to the most appropriate hospital in the area.

Your team collaborates to provide you with the best possible health outcome, and we are committed to providing you with a personal physician who attends to your health care needs and coordinates your care across all settings, including the medical office, hospital, clinics, behavioral health, testing facilities and other places where you receive health care.
