Classes & Events

The Family Medicine Center offers a variety of group appointments and classes. The current offerings include Diabetes Group Education Program, Burmese Outreach and Education Class, Childhood and Family Wellness Group Education Class, Diabetes Prevention Program and Shared Medical Appointment. Contact us for more information about these group visits or ask a staff member at the Family Medicine Center.

Welcome to The Family Medicine Center (FMC)!

We are honored you have chosen us to care for you. Our goal is to walk this journey along with you, giving the best medical care and support from the very start. We welcome all of your questions and encourage you to share your concerns (and your joys!) with us. Our caring doctors, nurses and staff look forward to helping you.

Our goal is to be your partner in health care by serving as your medical home. We are committed to make available to you a personal physician who provides for your health care needs and coordinates your care across all settings, including the medical office, hospital, clinics, behavioral health, testing facilities and other places where you receive health care.

FWMEP is a family medicine residency training program where each resident and faculty member manage their own patient panel. We use evidence based treatment plans to provide high quality care. We use a team approach to address all of your medical needs. Your integrated healthcare team is made up of clinical support staff, certified diabetes educator, physician, pharmacist, psychologist and social worker who collaborate to give you the best possible health outcome.

We are now accepting self-pay, private insurances, Medicare and Medicaid. The Practice provides equal access for all patients, regardless of insurance status. We can assist with health insurance questions and Medicaid applications.

We expect you to provide us with your medical history, as well as information about any care you obtain outside of our Practice. This includes your current medications, recent test results, visits to other health care providers, behavioral health services, hospitalizations, and emergency department visits.


There are a few items we need you to bring for your first visit:

  • Your state issued ID

  • Your insurance card

  • Any medications that you are taking (supplements and vitamins included)

Community members

Patient Advisory Council

The Patient Advisory Council is an important component of our Patient Centered Medical Home. It represents the diversity of our community and is comprised of 10-12 current patients and/or family members who serve for two years in a volunteer capacity. The Council meets once a month to provide structured feedback pertaining to the services rendered and care received at the Family Medicine Center.

Getting to the Family Medicine Center

Family Medicine Center is located in downtown Fort Wayne on Broadway between w. Main Street and W. Wayne Street. You can find us in Suite #350 of the St. Joseph Medical Office Building (which is the only building with blue awnings). We accept sameday appointments, for new patients as well.

If you are driving to our clinic, please park in the St. Joseph Hospital Medical Office Building Parking Garage, entrance on Broadway. All access to the Family Medicine Center from the parking garage is handicap accessible and will be validated for free by Family Medicine Center staff after each visit.

If utilizing the Fort Wayne Citilink Bus System, you can access the clinic from the bus route #1 Waynedale via Broadway/Northecrest. Downloadable .pdf maps in both English and Spanish are available at  

Entering the Family Medicine Center

Please enter the St. Joseph Medical Office Building from Broadway Street. Use the door which is the first blue awning next to the parking garage. Please take the elevator as soon as you enter the building to the 3rd floor. Once you exit the elevator, walk down the hall to your right. At the end of the hall veer to your left-the door at the end of this left side of the hallway is the entrance to the clinic.
