Patient Centered Care
Our goal is to be your partner in health care by serving as your medical home. We are committed to make available to you a personal physician who provides for your health care needs and coordinates your care across all settings, including the medical office, hospital, clinics, behavioral health, testing facilities and other places where you receive health care.
FWMEP is a family medicine residency training program where each resident and faculty member manage their own patient panel. We use evidence based treatment plans to provide high quality care.
The Practice provides equal access for all patients, regardless of insurance status.
How We Are Different
The Role of the Medical Home
We use a team approach to address all of your medical needs. Your integrated healthcare team is made up of clinical support staff, certified diabetes educator, physician, pharmacist, psychologist, and social worker who collaborate to give you the best possible health outcome.
Pediatrics, Obstetrics, Adult Care, Geriatric Medicine.
We teach patients how to self-manage illnesses such as diabetes, hypertension, COPD, back pain and obesity.
Counseling for lifestyle changes such as healthy eating and how to quit smoking.
Assistance with health insurance questions and Medicaid applications.
Mental health counseling
Now accepting self-pay, private insurances, Medicare and Medicaid.
What You Can Do
We expect you to provide us with your medical history, as well as information about any care you obtain outside of our Practice.
This includes your current medications, recent test results, visits to other health care providers, behavioral health services, hospitalizations, and emergency department visits.
You may request care and advice by calling at any time of the day or night.
